The below is an application to secure an exhibit booth at the PD Care Connection Experience to be held on Thursday March 26, 2026, through Friday, March 27, 2026, at the ASU SkySong building, 1365 N. Scottsdale Rd., Scottsdale, AZ 85257. To secure a booth, please complete this form below. Sponsors will be notified over email if we are able to accommodate your request.

Booth Information

Standard booth size is approximately 6' X 6', and some larger booths. The price includes table and drapes. PAYMENT of 50% IS REQUIRED WITH SIGNED CONTRACT. 100% PAID BEFORE BOOTH SPACE WILL BE ASSIGNED OR CONFIRMED. Full payment due by March 1, 2026. All payments received are non-refundable 48 hours after received.

The following sponsorship levels are available:

Primary Sponsor - $12,000+

Stage Sponsor. Name and logo at the top of banner board and prominently displayed in the venue. Name on marketing materials. Listed on the website as Primary Sponsors. Choice of location for table in venue. Breakfast, lunch and social. Cocktail hour sponsor. 10 raffle tickets.

Gold Sponsor - $8,000+

Lunch sponsor. Name and logo on marketing materials and displayed in the venue. Listed on the website as Gold Sponsors. Choice of location, after primary sponsors, for table in venue. Breakfast, lunch and social. 7 raffle tickets.

Silver Sponsor - $6,500+

Breakfast sponsor. Name and logo on marketing materials and displayed in the venue. Listed on the website as Silver Sponsors. Choice of location, after gold sponsors, for table in venue. Breakfast, lunch and social. 5 raffle tickets.

Bronze Sponsor - $5,000+

Name and logo displayed in the venue. Listed on the website as Bronze Sponsors. Choice of location, after Silver Sponsors, for table in venue. Breakfast, lunch and social. 3 raffle tickets.

Sponsor - $2,000+

Name displayed in the venue and on the website. Choice of location, after Bronze Sponsors, for table in venue. Breakfast, lunch and social. 1 raffle ticket.

Booth Vendor - $600+

This is for small businesses that service or have interest in the Parkinson's community. Name displayed in the venue, and on website. Limited booths at this level. May share a table in the venue. Breakfast, lunch and social.
